When a problem comes up in your workplace - what’s the standard procedure?

How does it play out in your company when things go awry?

Who is in charge of solving problems and / or issues?

Is this clearly established in your workplace - or a complete mystery?

Are major problems confined to specific departments, or is there cross department task groups set up to address them?

Is there a framework or system for problem solving in your organisation?

If a problem solving framework does exist - what does it look like? If not - how would you describe the current way of tackling problems?

Too often time and resources are spent on trying to fix the wrong thing. Does this happen much in your organisation?
